VOLUNTEER

Volunteers are valued members of our growing health care team linking us with the community, offering a broad spectrum of opportunities and meaningful experience. Orientation, facility tours and appropriate work placement is offered to all new volunteers. We proudly support the St. John Ambulance Dog Therapy Program, intergenerational programs, group and individual interaction, as well as volunteer opportunities in several departments' facility wide.

144 Bed Accredited Long Term Care Facility in Lunenburg, Nova Scotia

Care Navigator Position


Harbour View Haven is a not-for-profit 44-bed Long Term Care facility located in beautiful Lunenburg Nova Scotia. Licensed by the Department of Seniors and Long Term Care, Harbour View strives to provide person-centered care to seniors requiring support with daily living.

Residents receive multi-disciplinary care based on a holistic approach to ensure physical, mental, and spiritual well-being while balancing resident rights to individuality, respect, and dignity.


Key Responsibilities:

The Care Navigator role is intended to support residents and their families during a resident’s stay at Harbour View Haven. This includes guiding the resident and family through the admission process, transitioning to living in a nursing home, and ongoing support during one’s stay.


Qualifications:

• Completion of a minimum of a Bachelor's Degree in Social Work

• Completion of a Master's in Social Work or enrolment in a Master's program is an asset.

• Must be a Registered Social Worker in Nova Scotia or eligible for registration in the province.

• Must have at least one year of social work professional experience.

• Experience working with the senior population is an asset.


Responsibilities:

• Reports directly to the Director of Resident Care

• Facilitates admission and in-house transfers for residents in close collaboration with the care team as well as other facility departments.

• In consultation with the care team, conducts pre-admission and ongoing assessments of residents’ bio-psycho-socio-cultural needs.

• Addresses issues and challenges presented by and/or regarding residents and family members through individual support/counseling, group work, etc.

• Coordinates and facilitates multi-team meetings and resident/family council meetings

• Keeps accurate and timely documentation following facility processes and practices. Also, maintaining accurate statistical records as assigned.

• Plans and implements interventions for challenging behaviours in collaboration with internal and external resources.

• Leads or participates on various committees to promote resident and staff safety.

• Provide staff support and education as appropriate. Provide debriefing as required.

• Liaison with (but not limited to) Department of Seniors and Long Term Care, Nova Scotia Health, provincial placement officers, etc.

• Facilitates special needs requests for residents as appropriate.

• Develop informational materials (pamphlets, toolkits website diagrams, etc.) that will benefit residents/family members and staff.

• Familiar with advance directives, power of attorney, and other pertinent legislation that may impact residents in long term care. Able to assist families in the navigation of processes to identify these key processes.

• Participates in ongoing professional development and lifelong learning


Skills Needed:

• Resident-centred care as the core motivator for service delivery, understanding and addressing residents’ needs and concerns with empathy and compassion. Advocating for the best interest of residents.

• Strong understanding and advocacy of best practices in Nova Scotia healthcare

• Must possess the ability to deal with complex situations and make sound recommendations.

• Ability to network and develop strong relationships with team members and community partners.

• Possesses ability to work effectively with diverse populations and cultural backgrounds.

• Strong organizational skills to manage multiple tasks and coordinate care effectively

• Strong verbal and written communication skills essential for effectively interacting with residents, families, staff, and external partners.

• Proficiency in Microsoft products and other computer systems (familiarity with Point Click Care is an asset).


Hours of Work:

• 32 - 40 hours/week (to be determined with successful candidate)

• Workhours as needed to support residents

• Flexible hours per resident admission and meeting needs


RECREATION PROGRAMMER

Temporary Summer Position

35 Hours/Week, May 12 – August 22, 2025

Wage: $17.50/hour

Qualifications/Responsibilities:

  • Must be a youth aged 15 to 30 years
  • Interest, training or experience in therapeutic recreation an asset.
  • Responsible for carrying out various therapeutic
  • recreation programs to meet our resident needs and interests under the supervision of the Therapeutic

Services Director.

  • Assist with maintaining garden spaces.

Requirements:

  • CCA Certificate required or acceptance into a future CCA program.
  • LPN License or Graduate License.
  • Experience in LTC environment preferred.
